License by endorsement

Hello,

Can someone please prove me with the lin to the applicator for Hawaii EN license by endorsement? I feel like I am going in circle. 
I only can see one form and that is for by exam.

Is there a different form for applying with endorsement? That link I can only see the application for by exam.

1. Select "Nursing License"
2. Select "Nursing License - (without exam)"
3. Select "Requirements and License Application Form"
4. Scroll to the end of the document to access the "APPLICATION FOR LICENSE (ENDORSEMENT OR  WITHOUT EXAM) - NURSE"

For fingerprinting, trying to make an appointment. For department/division for which finger prints are being submitted, do I enter BRN Hawaii?
