Lactation Consultant - How to document hours?

Hi everyone!

I've scoured the IBLCE website and still cannot find a place that has a streamlined form for documenting hours. Is this something I create on my own, or am I just missing it on the site?

When I did it it was just a form you printed and filled out by hand. Now they have a fancy spreadsheet. :)
