I am a new grad that has been working at a snf for 4 months in an area where very few new grads get in to hospitals. I tried to apply to the local ltach's, (Vibra/kindred/promise) but they said they don't hire new grads and require vent experience.
Currently I plan to work snf for 6 months, get a job in subacute for 6 months, then try to get in to one of the three ltach's in the area, then use that experience to get in to a hospital.
Does this sounds reasonable, or does this not work how I think it will?
Also, I've had back problems ever since I worked here. Are subacute and ltach harder on your back than snf?

I've worked at a LTACH for about 4 years now and it can be hard on your back. Most of our patients require 2 people (or more) to assist with mobility and we have a policy to turn every patient every 2 hours. We also have a policy to get every patient out of bed twice a day (unless they are hemodynamically unstable). Honestly, if your end goal is to get into a regular hospital, I would focus on that. Apply for every job you can in the hospital. Then, once you are employed in the hospital for a while, you can look to transfer to a particular unit if you aren't in the one you really want to be in. I've been in the position of hiring people before and while I understand the hopping you want to do to build your experience, it could be concerning to a potential employer that you will just use them as another hopping point.
