how does it count when you retake it? Does the new grade replace the old, or do they average them together?

Here, they take the new one for GPA purposes, but the old one stays on your transcript with some sort of designation that it was repeated.

My school does as the above poster stated.As for nuring programs the program I am applying to only looks at the classes listed to make your GPA (meaning your new class). I have seen some programs which state that they recalculate your GPA to inlcude all classes ever taken including the new class, and the old class for admission. So in addition to knowing out your current college handles it make sure you check with the nursing program you are trying to get into.
