IDG Meetings?

I'm new at this and the training I got was horrible. I start on my own this week seeing patients, which I am okay with cause honestly it is no different that what I have been doing just less patients. I have not been told anything about these IDG meeting other than I have to go to one next week so I have to see all my patients and be ready by Thursday, but I have no idea what actually happens at these meetings...Can someone explain what I should be prepared to do during these meetings?

At our IDT (IDG) we give a brief history on our patient, provide an update on any changes, outstanding issues (DNR status, family issues, etc) and review the plan of care. Sometimes the MD has additional suggestions or gives new orders. Sometimes other staff members give suggestions on how to handle a particular situation with patient, family member or facility staff. Each member of the team participates, including the HHA, Chaplain and SW. Each adds their own "part" to come up with a complete picture of what's happening with that individual. At our IDT, we have to take computer notes about what was discussed (I hate doing this!!). The notes are then sent to the primary MD monthly to keep them in the loop.

I work for a hospice that has a very structured IDG and we have cheat sheets. We start with dx, comorbidities, any changes since last IDG, any spiritual or psychosocial issues, and end with why they are still meeting hospice criteria.

At our IDG meetings we state pt name; dx; LOC; symptoms to be managed and what we are doing to help with these; also any spiritual, family, social problems will be discussed; all RN's on duty, nurse manager, homehealth aides, medical director, chaplain and social worker are present
