are you looking to consolidate information or compile more information into one place?

we have an exstensive trifold that consists of i & o, drips, hourly vitals, lab values, vent settings, progress notes, prn meds & tx., assessments, etc.

A skin assessment section is really helpful. Ours had a little picture of a person, front and back where we could identify the exact areas of problems and indicate where lines and tubes were, plus there was a section for notes. It was really helpful to be able to visualize everything. Also, a separate line for blood sugar along with the vitals is helpful when you have insulin drips.

I actually don't work at the hospital that used that flowsheet anymore. The hospital I'm at now has a terrible flowsheet. Most of the time we have to cross out other things to write in what we need. It doesn't even have a place for O2 sats, but has a huge section for swanns that we hardly ever use anymore. Sorry I'm not more helpful.
