So a patient codes (let's say PEA) - they either open the chest or it's open already - they have an IABP - and they decide to to do ICM, what do you do with the balloon? Pressure trigger?

You would do the same as with external compressions: switch to arterial pressure trigger--then be sure that the ICM creates enough pressure to trigger the machine.

Keep in mind that AACN designates this as Level IV (Limited studies), though there is evidence to suggest that the IABP increases coronary and cerebral perfusion in a code.

I would have my friendly neighborhood medical or hospital library do a lit search for internal cardiac massage and balloon pumps.

I already checked the AACN Procedure Manual for Critical Care and it was not mentioned.

I would use the pressure trigger as a first line intervention. If that was causing problems, I would try the internal trigger (uncommon).
