how do I obtain my provider number in the state of new york

I am a LPN moving to New York. I currently live ive in ohio, I am a Independant provider I am my own boss. I am trying to figure out how do I obtain my provider number in the state of new york and is there a web site where the cases are? I have called every where and no one knows what I am talking about, but i called medicaid and new york does have this service. Please help anyone!!!
