how do I obtain my provider number in the state of new york

I am a LPN moving to New York. I currently live ive in ohio, I am a Independant provider I am my own boss. I am trying to figure out how do I obtain my provider number in the state of new york and is there a web site where the cases are? I have called every where and no one knows what I am talking about, but i called medicaid and new york does have this service. Please help anyone!!!:sniff:

Perhaps the Medicaid office in New York would be able to help you out and point you in the right direction. I'm sorry that you are encountering so many difficulties with this process.
