I need help about my case analysis

Hi im wilfred...
I am a 2nd year student nurse in Laguna, Philippines.. I have now a case analysis about PTB and COPD... Im not good in making a Nursing Care Plan and I dont know what to do in the Medical Managment about my patient..
I need someone to help me about my nursing care plan because my case analysis will be submitted at July 11,2005 Monday....
I also want to be informed more about how to make such Plan.. Thank you very much and Im hoping for your help...

Hello, kabayan! First you have to had a problem. I mean you have to assess your patient first( subjective; what your patient told you Ex. i feel tired and i can't breath,objective; what you observe like wheezing sounds and loud and prolong expiratory phase. :uhoh3: then, you need a nursing diagnosis for example Ineffective Airway clearance r/t increased mucus-secreting glands/thick secretions/impaired exhalation/smoking or Impaired gas exchange r/t loss of lung tissue elasticity. :uhoh21: after that you have to prioritized your intervention for example you have to auscultate lungs to note and document significant change in in breath sounds and then give the rationaleand then your final outcomes. I hope it help!

Thanks for that......it is a big help.. but my problem now is the only subjective i have when I interview my patients daughter is body weakness I need an objective, plan of action and some interventions w/ rationale... Im not good of making it cause Im still a newbie..^_^

Can you help me make an example...pls...

well, you suppose to ask the daughter if he eat a lot, when he start coughing,since she said body weakness,does he walk and then get tired easily. Objective is what you observe that's why you have to assess your patient, your instructor suppose to teach you how to do that. intervention is your action for example promote more effective breathing pattern for better gas exchange. rationale is the reason why you have to do that.Example high fowler's position will favor better lung expansion and more easy to breath.It will be more easier if you ask your instructor. they will help you a lot. good luck

Type in COPD nursing care plans and click item #3...a fair one....

Good luck. May try Mosbys Electronic Care Plans, too, but you would have to purchase this and not sure you have the time. Typical nursing student waits until the last minute to submit care plans (ha ha ha) :chuckle
