I am consueling a patient whose food reveal high in fat, calories and sodium. What would the client need of the risks factors on his diet on the future of his diet?

Unfortunately your post makes little sense. I got that you are counseling a patient who's diet is high in fat and calories and sodium. It sounds like you meant to say what would the client need to do to decrease his risk factors? And what recommendations would you make in the future for his diet? It's hard to tell what you are asking.

^ Agreed.
Most nursing textbooks will have a chapter on nutrition and nutritional management. Especially in the management of poor diet, such as you've tried to described (high in fat, sodium, energy etc.)
In short, what the patient needs is a dietetic consult and education regarding what constitutes a healthy diet and the risk factors of poor diet and obesity (heart disease, NIDDM etc)
