How to reapply for Nclex-PN California? Where to get a form to reapply?

I just need a little help here. I took my Nclex-PN (California) and didn't pass.

I would like to reapply.. Can anyone help me where to find the reapplication form? and how to reapply?

Fo real? I mean I just googled "how to reapply for the nclex-pn in california" and got what appears to be the answers

You need to apply again and pay the same fee after 45 days.

if you still feel unsure, you can contact your licensing board, they list email and a phone number, besides having a mailing list, twitter, newsletter etc.
