How much does your triage service charge?

Do you charge the doctor a flat fee for after hours triage service? What do you charge per doctor? Does it depend on the number of patients they have?

In my research so far I have found that triage services are charging per call $17-$18. ANd then a flat fee of like $300 per physician. is around $1000 a month I'm guessing.
