I'm looking for advice...I graduated (top of my class) and then worked nearly one year in L&D. I had to leave due to the need to have surgery and then spent two years healing and the last 4 as MOM. I'm well and really desire to go back, either part-time or per diem. My license is active and up to date. Am I hirable? Do I have to go back to school? I feel very confident, even after all this time, that I'm capable - with mentoring for a period of time of course. So, any suggestions for interviewing, or how to state my resume/cover letter? All replys would be very appreciated.

For your own sake, I'd look into a refresher course. Or a hospital that would be willing to give you a "re-orientation" period.
For the sake of confidence, you need to have cushion where expectations are concerned. Yes, you have experience and your skills may come back easily, but you also have to consider that your comfort level may take some time to return.
I was out of nursing for five years and took a refresher at a technical college last October. It was inexpensive, short in duration (8 weeks), and absolutely on target and useful. I had no trouble finding a job once the class was over and feel very much "at home" in my new setting.
I'd recommend a refresher course for another reason. You are now juggling more personal responsibilities than you were when you first started working. Going to school is a good way to learn how to balance your various committments without worrying too much about screwing up in a job setting. Taking a course should give you some insight into how much time you'll need to commute, what priorities you want to set, and how to arrange for good, dependable child care. Think of it as a clinical for being a working mom.
Plus it looks great on a resume.
