How to get new york endorsement for RN

I have RN license from Michigan and recently moved to New York. what is the procedure to get an endorsement in NY? also from where i can download the form for endorsement in NY

FYI: You NEED to go through CGFNS (http://www.cgfns.org/) too if you didn't get your nursing education in the U.S. i.e. you attended nursing school in Canada. Yes, this is the case even if you have a RN license from another state.

In question No 18, do I need to specify the registrations that I got from out side US or just Nclex-RN that I got from Michigan?

oh uhm i'm also trying to get my NY license to work up in plattsburgh. i have the packet and it asks for the dates which i started and ended elementary school and high school? is that really necessary cuz i have no way of figuring that out...

you can put down the month and year. that's what i did.
