How to become an independent CCM

Hi all,

     I am a RN with a BSN, I received my RN at the ripe age of 50 and my BSN at 53. Over the last couple of years I have developed bilateral dorsal pedal arthritis with cysts. I am looking forward to the future of not being able to do direct care and making a career change. My passion is to ensure that pts receive nursing, equipment ect that they are entitled to. Currently I work with 2 clients that are seen at major world renown hospitals in our area. As voiced by family,  they were unaware and not told of the services available to them. I am looking to become a remote independent CCM with a company that hospitals refer pts to, any suggestions ?
