Hello everyone! I was wondering if anybody could assist me with information on a good resources for home care triage. It seems our agency is in a little rut when it comes to after hours calls. I am looking for something that will direct the nurses on protocols for different problems. With the big focus on reducing emergent care and hospitalizations we must find a way to treat problems and get patients to the doctors office safely. Is there a tool that might offer guidance according to patient symptoms as to the level of acuity? How do your protocols guide you?

Home Health Triage is VERY different then Telephone Triage. YOUR HH Company has the option of going out rather then sending into an ER or primary care physician.
