Home health as an LVN

Hi, I am new  to Home Health as an LVN, I am currently trying out LVN Shift Care 8 hour one on one care, I am looking for Home health specifically LVN Shift Care additional notes (head to toe , narratives about the shift ) examples or temples, I feel completely lost , I read other notes from other nurses  and some are simple some even give me concerns and some are very informative , Is the plan of care a guideline to write out  narratives about the shift? I am in the So cal  area , any type of input that can help I would greatly appreciate it thank you in advance ☺️

I am considering going into home health health as an LPN.  I previously worked in nursing homes and private duty.  I bought a book called Chart Smart when I first started.  It really helped me learn how to chart better.  Good luck.
