Hello, HHC nurses. I would like to add one other option to the e-mail reply to this survey: you can call me. If it is more convenient, I will provide you with both my home and cell phone numbers. This way you could call me with your responses. If you have viewed this survey, then you will notice that there are no personal questions such as name, name of workplace, etc. I will just ask you the eight questions on the survey and mark your responses on the survey. That's it. I am working with two nurses who are currently employed in HHC. They will poll HHC nurses at their respective places of employment, whereas, I will poll from, hopefully, this forum. This survey is an effort to explain why HHC nurses choose to work per diem versus full-time (or vice versa). Please contact me through this forum if you are interested in calling-in your survey responses. Thank you. I will check in periodically for your responses.

Sincerely,
