Hepatitis B Re-vaccination

When I have changed jobs in the past I have been asked to show proof of prior Hep B vaccination. This was accepted without follow up. This time when I took a new job I could not find the old piece of paper. They did a titer and found basically a 0 titer. I then was told I would have to have the entire series repeated. Has this happened to others?

Shouldn't I have been offered repeat titer after #1 injection to see if I just needed a booster?
