hi, milly81, and welcome to allnurses! :welcome:
i would imagine that what your instructors are wanting you to do is develop your own template for charting an assessment on ob patients. some of this assessment sounds like it is in the vein of gordon's functional patterns or something very close to it. some of it sounds like maslow's hierarchy of needs. i'm listing three links to guidelines to assess by gordon's that includes the kinds of questions to ask the patient/client for those particular categories:
do you have a copy of taber's cyclopedic medical dictionary? there is a 6 or 7-page nursing assessment form in the middle of the dictionary under the listing of "nursing". it will have the items you need to address under most of the categories you have listed that you need to assess as well.
you probably want to look at a reference on erikson's developmental stages in assessing the patient's growth and development part of this exercise.
there are two links to really good webpages on the assessment of the pregnant client from the health assessment resources, techniques, and forms thread which you can also see at: https://allnurses.com/forums/f205/health-assessment-resources-techniques-forms-145091.html. it is a sticky on this forum. sticky threads always appear at the very top of all the thread listings.
there is information on how to write a narrative nursing note on these threads or posts on allnurses:
