Help me please! My pt has hyponatremia. The doc's are running some more tests, but they think it is related to Syndrome of Inappropriate Antidiuretic Hormone (SIADH). He has been hallucinating. I haven't actually met my pt yet and his chart was gone so my info is incomplete to say the least. B/p's have been w/in normal range, edema? NANDA diagnosis? Interventions?

This is a patient you will have tomorrow in clinicals, right?
Look up the symptoms of SIADH and use them to determine possible nursing diagnoses until you actually get a chance to see and do a physical assessment on this patient. For hallucinations you can use Disturbed Thought Processes. Interventions are based on the symptoms that support the diagnoses.

My very first care plan is due monday and my hypocondriac patient was diagnosed in the ER with hyponutremia. I totally understand the frusteration and the confusion.
