Help Understanding Upper Arm Graft Cannulation

As a PCT in renal care, THIS procedure has been the most difficult for me to understand. I have had little difficulty cannulating straight fistulae or loop grafts within forearms, but these grafts give me and the patients the most headaches. I need someone to explain to me what I'm supposed to be looking or feeling for. What artery and vein is the graft connected to? I have researched and have found nothing on the internet concerning this, and co-workers seem to be of little help explaining HOW to stick this access.
