im a junior nursing student here in the philippines and need help. as part of our curriculum we need to have a case study, my patient is 73 years old female diagnosed with acute gastroenteritis (mild) her physical assessments are normal including her vital signs, what nudsing diagnoses could i do with her case.. im using a NANDA but so hard to make one.. cant think of any cause her's are all normal.. thanks and look forward for help:loveya:

I walked around with gastritis that developed into gastroenteritis for 6 months and was having all kinds of mild GI upsets that I wouldn't take to a doctor, most notable was the diarrhea. And I went through a lot of remedieis to do something about it.

she has no acute pain? Why did she seek medical attention? That's where you get your ND. Also sounds like a poor patient for a case study; I mean how long is she really going to be in the hospital with gastritis? The PP talked about walking around with it untreated for 6 months.

thanks.. the patients, seek medical attention because she felt nauseated and felt like being to vomit, but, as when we assessed her using the 13 areas, we didn'find anything wrong. she said she was tired that time and skip dinner and just took her medication for HPN which we thought taking medications with empty stomach causes irritation in her gastric lining that make her feel nauseated and was like to vomit..we also noted that her fingernails are dirty and might be one of the causes of having acute gastroenteritis. we made nursing care plan for hygiene. what makes the worst most is when the patient does'nt want to tell the truth. anyways, thanks for your response, and now i know h\what to do the next time.. thanks a lot and looking forward for you advise when i need..

what you assume might be the underlying cause of her problem is not the same as the problem itself. you were trying to find evidence of gastroenteritis while other gi symptoms were there, but you were ignoring them. you have to deal in facts. because you assumed "taking medications with empty stomach causes irritation in her gastric lining that make her feel nauseated and was like to vomit" you missed a valuable clue--nausea. she also "skipped dinner because she was tired". another clue. you missed these symptoms of nursing diagnoses because you were assuming other information and ignoring actual facts. we do not play doctor. we hunt for clues of nursing problems and treat them. if we pick up evidence of a medical problem all we can do is report it to a doctor, but we treat the patient for the nursing problem.

follow the steps of the nursing process when care planning.

step 1 assessment - collect data from medical record, do a physical assessment of the patient, assess adl's, look up information about your patient's medical diseases/conditions to learn about the signs and symptoms and pathophysiology - you needed to look up gastroenteritis. in fact I gave you weblinks about it.

step #2 determination of the patient's problem(s)/nursing diagnosis part 1 - make a list of the abnormal assessment data

• nauseated, felt like vomiting
• tired
• skipped dinner

step #2 determination of the patient's problem(s)/nursing diagnosis part 2 - match your abnormal assessment data to likely nursing diagnoses, decide on the nursing diagnoses to use

• imbalanced nutrition: less than body requirements
• nausea
• fatigue
