Hi, I was wondering if anyone could help me, for part of my assesment of practise document I need to make an action plan, I have a week to complete it, but I don't know how, my Uni Supervisor is on anual leave and the next apointment I can get with another tutor is in a fortnights time!!!!!!!!
Does anyone have any tips how to set one out? Or know the location of any examples on the net?

Thanks, but I didn't mean a care plan, allthough I am going to keep that link for my next module:D I do have a basic idea how an action plan should be set out, and i quote from one of the books in the libary, the only one that mentioned actions plans!:
"a set of steps outlined to reach a specific goal, and should be set out in the recognised format."
My problem is I don't know the recognised format, If it is bullet points, arrows, a flow diagram?
And to answer your question dgh, the action plan, is on how I intend to meet the learning out comes of my placement, such as taking manual TPR's and BP's, as well as the electronic versions, and over comming any barriers to effective communication.
