Help with Student Care Plan

ok so heres my issue:

i need help coming up with a care plan...i have the functional pattern, label, etiologies, s&s im just having a really hard time coming up with outcomes and interventions.

my label is disturbed body image

etiologies: psychosocial secondary to weight gain and surgery secondary to lipoma tumor

i dont really know how or where to start =[ any advice or suggestions would be greatly appreciated!

You always want your outcome to be opposite of your problem, so if they have disturbed body image you would want the outcome to be a positive body image, along those lines. Just like if someone was having respiratory distress, your goal would be something opposite of that. Whatever would be a reasonable goal. If someone is having pain and rating it an 9, your goal would be to reduce they pain to a 5 in the next 2 hours, then further then that. Goals should be SMART

specific, measurable, attainable, realistic and timely

What kinds of things would you do to help someone that was hating the way they look because of weight gain?

I would say the number one would be getting them help, referral for counseling, maybe education on what they can to do battle the a little of the weight and so on.

i have my goal as: patient will have a positive self-image and increase in dietary knowledge

but after that were supposed to write outcomes directed by the etiologies. one of them has to be pt actively involved in learning to help resolve the diagnosis and the other has to show a change in signs and symptoms

So are the outcomes supposed to be real or made up?

If you are supposed to be making them up, you would say something like, patient is open to getting a referral and has agreed to make dietary changes and increase activity to improve weight loss.

Goal would be, patient spoke with counselor and started changing eating habits and increased activity and is eager to continue and shows a much more positive outlook on her situation.

Something along those lines.

A person with a disturbed body image is not going to have overnight goals and solutions. It will be an ongoing thing and initially a realistic goal would probably be, like 1 week or something for seeing some progress with attitude and starting the other phases.

PsychoSocial issues aren't resolved in a day.

She might also need a consult for a form of anti depressent for short terms to help her through all of this.
