Dear Director of Nursing,

I am writing to submit my resignation from my position as CNA in Community Hospital, effective July 1, 2007. I thank you for the oportunity to have served our patients in this capacity.

Sincerely,

Jane Doe

It is NEVER necessary, and rarely helpful to include any details in a resignation letter. Good luck in finding a position that suits you better!

Thanks Jolie, I've been working on my resignation letter and have been rather stumped too. This helps a lot.
