help with peds careplan?

Hey all,

I just got report (from my classmate) for my pt tomorrow who I haven't seen yet, and I'm pretty stumped on coming up with my two nursing diagnoses. : /

So my pt is an 8 month male; the diagnoses was abnormal EEG w/ delayed visual maturation. Presented Dec. 2011 with spasms of the eyes and irritability and deviation of the head. The spasms have increased in frequency in all 4 extremities and duration is varied. Onset was Nov. 2011; noted loss of skills and regression.

I just have one lab so far: Hct which was high @ 87.1

Vitals were normal @ 94/74; 36.4; and respirations 30

So far, I've come up with Risk for delayed development r/t vision impairment (not sure what else to add to this??) , Risk for injury r/t muscle spasms..how about ineffective cerebral tissue perfusion AEB loss of skills and regression? Any advice would be appreciated. Thanks !
