Hey all, I just got report (from my classmate) for my pt tomorrow who I haven't seen yet, and I'm pretty stumped on coming up with my two nursing diagnoses. : / So my pt is an 8 month male; the diagnoses was abnormal EEG w/ delayed visual maturation. Presented Dec. 2011 with spasms of the eyes and irritability and deviation of the head. The spasms have increased in frequency in all 4 extremities and duration is varied. Onset was Nov. 2011; noted loss of skills and regression. I just have one lab so far: Hct which was high @ 87.1 Vitals were normal @ 94/74; 36.4; and respirations 30 So far, I've come up with Risk for delayed development r/t vision impairment (not sure what else to add to this??) , Risk for injury r/t muscle spasms..how about ineffective cerebral tissue perfusion AEB loss of skills and regression? Any advice would be appreciated. Thanks !
  2. hey y'all : ) just needing some advice on how to handle this situation.. well, to start off my grandfather is 83 years old; my grandmother had called me saying he fell down in the bathroom today while I was out today ... he apparently didn't have his walking cane on him at the time. when my mom and i rushed back home, he acted like everything was normal, though when we asked him, he couldn't remember anything from when he fell down (btw he has some mild dementia, he doesn't have much of a good memory anymore). we examined the cut : its cuts across his left temple, above and very close to his left eye, it looks superficial, but there was still some mild bleeding involved and it looks swollen/bruised. my mum and i wanted to take him to the doctor, but he refused saying that he is alright. he says there is nothing wrong with his vision in the left eye. so my questions are: how do i take care of him, how can i tell if he suffered something more serious like a concussion or something, and should i take him to the doctor since this is a head injury on an elderly man? im sorry, im just a clueless first year nursing student hah.. but i just want to make sure that this is nothing serious and that he hasn't suffered some serious damage ... help and advice greatly appreciated ! :)
  3. I just finished up my finals for first semester for my associates in nursing. I squeaked by in my other three classes, but in Pharm, my final grade at this point is a 799 and I need an 800 to pass. FML, right?? my professor emailed me to call her later on this evening to discuss my grade situation , but I honestly have no idea if she will pass me or require me to take pharm again. but in her emails, it sounds like she's already exhausted all the extra points to give me. Why can't she exhaust ONE MORE FREAKING LOUSY POINT?? i admit i struggled in pharm (the tests killed me) and i honestly feel like i haven't retained any of that information. there was just so much information to memorize and it was so overwhelming for me. maybe it would be best for me to take a break off this upcoming semester: re-take pharm, get all the basics down, and start preparing for the NCLEX. but I don't want to let down my parents/friends/family by having to retake a class again. everyone has had so much faith in me, and i feel like a failure for flunking a class my very first term. what do y'all suggest? should i fight for my teacher to give me that one extra point to move on to term 2? or should i agree with her and retake pharm if she suggests it? ~thanks everyone~
