Help with patient goals for risk of infection

First year nursing student working on care plans.

I could use some suggestions for patient goals related to a nursing diagnosis of "risk for infection". My patient was admitted to the hospital for a mass in colon. She ended up having a transverse colon resection. Patient is 90 years old. Not tolerating clear liquid diet and NG tube was reintroduced. Patient was ambulatory before procedure but is now only capable of transferring out of bed to a recliner with one assist. She has a slightly elevated temperature and pulse and CBC shows low levels RBC HGB and HCT. IV fluids were also reintroduced and urinary catheter still in place. My instructor is looking for care plans to be in the form of; nursing diagnosis, followed by one long term goal, three short term goals, and 5 interventions with rationales. The intervention part has come easily but my long term goal of "Pt. will show no signs or symptoms of infection through hospitalization" is making it difficult for me to find short term goals for my client to meet. Can any one offer any suggestions? Thanks so much in advance

see https://allnurses.com/forums/2751313-post8.html. interventions are to prevent the occurrence of an infection. the goal, or anticipated result of that is that no infection will occur. that can be expressed in many ways.

  • "pt. will show no signs or symptoms of infection through hospitalization"
    • break down the "signs or symptoms of infection"
    • pt will have a temperature with a range of ___ to ___ throughout hospitalization
    • pt will maintain a wbc count of ____ by discharge.
    • incision will be without redness, swelling or purulent drainage on ____.
