I need help with this nursing diagnosis...

Nausea r/t multiple etiologies AEB statements of 'I feel sick' and vomiting...

is this correct??

the etiology bother me as you have it written. if there are "multiple etiologies" and you know them, then say what they are. this webpage for this diagnosis list some of the etiologies that can apply if you need help describing them:

with evidence it is customary to sequence the objective data first and then the subjective data. so, "aeb vomiting and patient's statement that "i feel sick'" would be clearer if the patient said, "i feel like i'm going to throw up." that's a clearer statement that they are nauseated. there are other symptoms of nausea that can be observed in people who are nauseated or the patient may describe to you:

  • gagging
  • increase in salivation prior to vomiting
  • increased swallowing (of the increased saliva before vomiting)
  • a sour taste in the mouth

you did a good job of constructing the diagnostic statement and getting all 3 parts correct and in their right order.

Thank you...it's mainly due to his medications, morphine and antibiotics. he also experiences a lot of pain as he has scc of the neck and is palliative

Then you need to say that: Nausea R/T side effect of medications AEB [symptoms of nausea you observed], vomiting and patient statement that "I feel nauseated."
