Hello guys,
I'm currently processing my application for CVS in New York State. I choose to apply online as well as pay it thru credit card online also. I have already printed the required documents such as CVS- Authorization for Validation of Registration/License as well as the CVS- Authorization for Verification of Academic Records/Transcripts and the Application form. I am a bit confused because there are blank lines in the form, should I fill them up in ink or leave them as it is. Also in the Validation for Registration/License, it asked for the Licensing Authority's name and it's address, since I am living in Cagayan de Oro City, should I fill this up and put the address of the PRC here or leave them as it is? I have also noticed that the attendance date is different from what I filled up online since in the printed form it reflected the exact date of the attendance date...I'm really confused about this matter guys, and I hope someone who experienced the same situation as mine could help me shed some light. I am aiming that before I send those forms, there will be no error in order to prevent the delays in the processing of my credentials...Thank you in advance guys!

When I did my application, I did not write anything on the blank lines. For the licensing authority I think you should write the PRC's address in Cagayan de Oro :)
thank you for the reply...=)...okay so I'll just have to leave everything in blank and write the PRC's address in ink just like you did in your application...the steps are really confusing and I'm soon to be out of my wit's end already...hahha..and also another thing is when I already pay it thru credit card, do i need to send the payment voucher or not anymore?
