Help - Focused nursing assessment

I'm a student nurse who just came off a 7 week placement on a cardiology ward. On our placement we had to complete one focused assessment, obviously I did a cardiac assessment.

I'm wanting help for how I should write this information out :shy:.

Our lecturers want us to type out our focused assessment in a word document and I've been trying to find a template but I can't find any. I have all my information but I just want to know the best way to write it all out.

Any help is much appreciated

Do you have a physical assessment textbook? Do you know what a SOAP note is? We will help you but you need to show us what you have so far. Can you tell me the difference between objective data vs subjective Dara?

I'm a student in New Zealand so there may be some differences, no sorry i have never heard of SOAP ( the acronym :roflmao:) Yes ive got my subjective data and objective data

I have all my subjective data and my objective data that i got from the patient and files all scribbled down a numerous pieces of paper. I just want to know a good way to present it.

Would I give a general overview of the patient then have that flow into my subjective data then have that go onto my objective data. I've been going by my "Jarvis's Physical examination and health assessment" text book and "Lewis' Australian-New Zealand Medical Surgical text book", but again that doesn't tell me how I should write out a focused assessment it just guides me to the questions I should ask and also shows how to do perform a physical cardiac assessment.
