Help with documenting?

I was hoping someone could give me some bullet points on documenting (a Nurses Note). I would like to know the basic outlines that must be used in every document.

I have done Nurses notes before but I always feel mine are inadequate and lacking quality and the information that needs to be relayed about the patient.

So in conclusion I'd like the foundations that must be in every note and perhaps the basic/commonly used formats and outline for nurses notes/documenting.
