Help with care plan for school!

I really need help with care plans, specifically for this patient. Give me a CHF patient or COPD and I'm good but for some reason, I cannot for the life of me figure this one out. I think I am getting so tripped up on the request of it being for two different systems.....especially b/c I feel there isn't a whole lot of objective data, its hard for me to actually picture this pt.

For GI? possibly

Obesity r/t excessive intake in relation to metabolic need/physical activity (caloric expenditure).

And then

Cardiovascular

Risk for impaired Cardiovascular function r/t family hx of cardiovascular disease; hypertension; dyslipidemia; insufficient knowledge of modifiable risk factors; obesity; and sedentary lifestyle.

The care plan is for two separate priority nursing diagnosis and two different systems. (Neuro, resp, cardio, GI, GU, Skin and MS). You must be able to explain the pathophysiology of what is physiologically occurring that causes THIS nursing problem.

GI: deficient knowledge, imbalanced nutrition

CV: Ineffective Tissue Perfusion or Dec. Cardiac Output r/t increased preload or increased afterload from HTN, Obesity, possible atherosclerosis? Possible DMII?

MS: activity intolerance

Resp: Possible oxygenation or airway due to his obesity (apnea?) are his O2 sats low?
