  1. I really need help with care plans, specifically for this patient. Give me a CHF patient or COPD and I'm good but for some reason, I cannot for the life of me figure this one out. I think I am getting so tripped up on the request of it being for two different systems.....especially b/c I feel there isn't a whole lot of objective data, its hard for me to actually picture this pt. I was thinking For GI? possibly Obesity r/t excessive intake in relation to metabolic need/physical activity (caloric expenditure). And then Cardiovascular Risk for impaired Cardiovascular function r/t family hx of cardiovascular disease; hypertension; dyslipidemia; insufficient knowledge of modifiable risk factors; obesity; and sedentary lifestyle. The care plan is for two separate priority nursing diagnosis and two different systems. (Neuro, resp, cardio, GI, GU, Skin and MS). You must be able to explain the pathophysiology of what is physiologically occurring that causes THIS nursing problem. Lucas Moreno is a 56-year-old Hispanic man who arrives at his employer's health occupation clinic for his biannual health evaluation. Although he appears to be in general good health, his current weight is 25% above the ideal weight for his height and frame. His triceps skin fold measurement is 21 mm (normal is 12.5 mm), and his cholesterol level is 245 mg/dl (normal is When you bring up the subject of weight and lack of exercise, he is not opposed to discussing the possibility of losing weight and increasing his exercise. Lucas states, "I would like to learn more about good nutrition." What else does Lucas need assistance with?
