Hello everyone! I didn't see that anyone posted about CONAH Fall 2025 application yet so I wanted to start one. If anyone has any information that they want to share or questions, we can all update each other on here.

Do you know if it's just that 13 page form or is there another form we have to print and fill out prior to the health screening

Pretty sure it's just that packet. I plan to call tomorrow to confirm. I'll post on here if there's an update 🙂

Update- I called & confirmed that you only need to fill out the packet included in the email. The health clearance and live scan (background check) must be completed before the first day of orientation. I was advised to finish everything asap to avoid any delays. 

Did you guys also fill out the respiratory fit test portion of the packet?

I didn't have a lot of it filled out when I went and I think they do the respiratory fit portion when you're over there

Oh really? Even the TB test portion? I just have my vaccines from my childhood too not much else.

Yeah, I got my TB test done over there. I mostly just filled out the parts that required my name and general information 

Hi everyone, I went in today but didn't fill anything out ahead of time because I didn't want to risk doing it wrong. They were super nice and actually highlighted everything I needed to complete, which was really helpful. I only brought my vaccination records from when I was 18 until now, and that was totally fine. They also drew blood and TB test while I was there.  Now I just have to wait to get cleared, and then I'll be able to go in for the Live Scan.

For those who got the health screening form how long did it take you after submitting the online information form? I submitted it since I had a couple questions and spoke with them on the phone already but wonder how long it takes to receive the health screening form or info on live scan?
