Goals and outcome criterion in Care Plans

Here I go again....test on Monday and I'm a bit confused. My text doesn't have much at all about outcome criterion. In fact, they don't mention it, but they do mention desired outcomes. The same thing? Who knows. ..... so, I'm asking all of you...

In writing care plans, what is the outcome criterion in relationship to the goal?

The outcome criterion is how you know if your goal has been met or not. The "as manifested by" (AMB) or "as evidenced by" (AEB) part of the nursing diagnosis. Say your nursing diagnosis is "pain." Your goal could that your patient will have a reduction in pain. How are you going to determine if this was done? Lets say that you will consider his pain to be reduced if he rates it as less than a 4 on a scale of 1-10, he is able to participate in his ADLs, there is a reduction in his moaning, etc. You can have one outcome criterion or you could have several.
