I need some help. I am the ADON in a rehab, SNF facility. The MDS nurse puts out a list of whose assessments are to be done when. When the assessments need to be moved to capture higher levels of care, she crosses off the old ones and writes the new dates at the bottom of the list. They are getting missed, I think because the nurses look down the list and stop at the oldest date on the original list.

Does anyone have a suggestion as to how this can be done more diligently so that we do not lose money because the assessments are late. I have tried everything I could come up with and nothing seems to work.

Have the nurses highlight the assessments as they are completed, then any left still need to be done. Another method would be to have an expand-a-file with numbered tabs and file the blank forms with only the name on them filed the week before they are due (or however long they will have to work on them) the MDS nurse can just move them if the completion date changes.

I only put out the assessment assignments one week in advance. I assign them to the nurses on different shifts. They have to turn in the sheet to me. But you can't just put out the list and leave it. It should be checked every day to make sure that it is being done. I tape the assignments to the cover of the 24 hour report book. PMs are highlighted in green, AMs are highlighted in blue and the NOC nurse portion is highlighted in pink.

We only do ours a week at a time. Any changes are sent out via internal email. We are a tight knit group and really stay on top of "The List".
