Nursing Wellness Diagnoses?!?

I'm working on a Health Promotion paper that requires 2-part nursing wellness diagnoses, and I'm not sure how to do them. The teacher also wants us to prioritize them and write out a rationale, but with it only being a 2-part statement, how do you do that? We can't have an AEB statement so I'm confused. We're supposed to look at this book by Stolte, but it's confusing and not making it easy to figure out. Then we have to chart it like you do with regular nursing diagnoses in using "planning, implementation, and evaluation."

Does anyone know how to do this????
