How do I fill out application form?

Hi everyone, I hold a RN license, work as a RN in a clinic(within one year). It is my first job, at that time this was a LVN position, so my started pay was low. Now I try to apply a new job. but application forms always ask these questions: can we contact your employer? your position? start salary? How can I fill out these questions? Any suggestions appreciate.

you can give permission for them to contact your employer so either say yes or no. if it's an old job that i don't work at any more i put yes. if i'm still working at the job, i put no.

write in the title of your position. don't use abbreviations, unless there's not enough room to write

what was your hiring salary when you first started the job. and you just put the amount. you can write it out per hour, month, week or year, etc.

As far as the salary I would put "negotiable". Take a look at new grad RN pay in your area and you will have an idea where to start.

i agree with DManAZRN about putting "negotiable" as salary. i was assuming that the "starting salary" blank was related to your job history.

Thanks everyone. especially phs 2000 gave me a lot of details! How about my position? Is RN or LVN? I'm still working at the clinic, if he know I'm going to leave, he may get mad.
