Sorry for incomplete post. The order was avastin infusion: dose 5mg/kg; calculated dose 400mg; diluent/route: in 100ml NS IV over 60-90 min. my concern is that calculated dose is much greater than order dose ( over 20% ). Should I back to MD or just do it?

avastin 5mg/kg, 400mg for infusion today. Pt weigh 143lbs. what are you going to do? Go ahead star infusion? or back to doctor? Thanks for your comments

Thank you for all of your responses. There is no standard in this clinic. when I taught my pt's family to do daily care only and one of the reach MD question me and other RN stated she used providone while she worked in the hospital.

Will you use providone iodine to clean a long term Foley catheter daily? would very appreciate your discussion and/or opinions.
