assessing quality of NH, SNF, AL, &c?

A friend of mine who lives in Oakland CA asked me about how to assess the quality of nursing homes. Her mother is ill and needs nursing care. She said she has neuropathy and can't get around; other than that I'm not sure what her limitations are. I'm not sure if she'd need a nursing home, skilled nursing facility, assisted living, or what. Is there a place where one can get ratings on quality of care for these types of places? Recommendations? &c. I have no idea where to look since I don't work in geriatrics. Thanks!
