Besides Focus Care (not impressed with them) is there another vendor who handles case/care management for Humana? I live in South central PA and the closest Senior Bridge office is 5 hours away. Is Focus Care the only one who works this area or are there others?

Are there other insurance companies who offer the same type of program to clients? It seems right up my alley...after 22 years doing skilled visits I am looking to rachet down a few notches! Thanks!
