First I'd like to thank all of you who lend support in this arena.

I have so many questions regarding the "final decision" from my board, please bear with me.

NO drug or alcohol treatment is required. Although my mental health professional/or MD must assess me before giving narcotics. etc.

The board does order me to abstain from alcohol and of course a given. Controlled substances. BUT alcohol use is lifted "if expressly permitted by his treater".

Drug and alcohol monitoring is required. Production of urine, blood, sweat, fingernail, hair, saliva etc...

Must practice in a facility under supervision. All quarterly reports etc.

Let me state here I have been requesting information from the board but have had NO response to my questions. In the interim. I have lost my job. Required surgery for a double hernia and have gone through the maze and frustrations of being uninsured needing physical as well as psycological care for depression. Which lead to this whole mess to begin with.

The case manager at FirstLab has been helpful BUT she is also waiting to hear from the board as to which tests will be required.

Have been unsuccessful in finding a job, any job to live, and most importantly follow orders from the board and pay for it!

Did not renew my license for my L.P.N. in April as I had no money to do so. Much less live or initiate and comply with these orders from the board.

The case manager from FirstLab has been helpful, but she in turn is waiting to hear from the WI. board as to what testing will actually be required. I have no luck with the board as I have said, but I asked her to contact me when/if she does of course.

So for the questions.

Should I assume that I will be required to give a total of 6 specimens or panels on a required day? (I need to budget for all of this.) Or a urine only. If positive then the others?

The collection fee. Is it a flat fee for a single drop day as a whole? Or is it a fee for EACH specimens given? As I write I am still awaiting a response from the lab in this matter.

The irony of my treater to "allow" alcohol use if they permit, being able to administer narcotics, not being required to do AA/or Drug treatment and yet being required to do testing. The other orders/requests by the board I have no questions about. Yet does anyone see the irony here? It seems nonsensical.

Any insight is appreciated. Thanks guys.

P.S. how to light a firecracker under the board and get a response would be helpful. I have saved all emails in this regard.

Typically health professionals provide a single split specimen urine sample (60cc) that is tested for I believe 12 items with FirstLab. (labcorp is the actual lab used by FirstLab for testing last i remember) Cost is generally around $50...it covers the entire 12 panel. The drop site will send it out to a special LabCorp lab that does all of the health professional monitoring tests. Results are back within a week. you will be notified of results ONLY if there is an issue.

There is no real need to "light a fire" under the board. As long as you have made the request for information, and the board has not told you to go test without telling you what for, there is no rush. You can only do what you are told, so wait to be told.
