I got this test question which made me confused:

16 y.o. pregnant girl tells nurse that her diet consists of

Breakfast - cereal + milk

Lunch - pizza+soda

Dinner - cheeseburger+milkshake+fries+salad

What's the most accurate nursing diagnosis:

1. Altered nutrition: more than body requirements related to high-fat intake

2. Knowledge deficit: nutrition in pregnancy

3. Altered nutrition: less than body requirements related to increased nutritional demands of pregnancy

4. Risk for injury: fetal malnutrition related to poor maternal diet

According to the book, the correct answer is 3. The rationale is that 4 (which I chose) is not correct because "there is adequate number of calories to support fetal growth". Ok, I could understand if they said, it was a potential problem rather than current, but they didn't... so... do fetuses need minerals and vitamins, or only glucose?
