fasting lipid panel on a pt with cont' tube feeding. ?h

A pt is on a continues tube feeding. Heard he has an order of fasting lipid panel in the a.m.

Also a diabetic. How is this pt suppose to fast!?

Hold the feed for however many hours is required. The lab should be able to tell you minimum number of hours has to be fasting. May need to adjust insulin if they're on long-acting ones.

Curious how that order pan out. He received 8o u of lantus at hs and gets routine coverage of reg ins

At midnight. I would think orders to hold or adjust ins should have been put into place before learning that this lab was scheduled in the a.m. this is in ltc.

Just FWIW.... the long acting should not be held (people who eat normally take hs Lantus, and don't eat all night); the hs sliding scale may need to be adjusted- but all of this with orders of course.

This would be a definite get clarification situation. If lab draws, it's useless. If the doc doesn't have labs, he/she is p-o'd. I would have called when I got on shift and saw the order. Don't care what time it is.....next time the doc needs to write a clear order re: the tube feedings, insulin changes, and noting some awareness of the patients continuous tube feeding. :uhoh3:
