ESI 5-Level Triage QI Audit tool needed

We have established an ESI 5-Level triage system in our emergency department, but I am looking to an audit tool that has successfully been utilized to triage level accuracy. I have seen numerous narratives accounts, but not an actual tool that has been created. Any help I can get would be greatly appreciated.
