endorsement and end-shift report?

Can anybody help me about endorsement and end-shift report topics.. I really need it because I'm going to report it in our class. I need to find its purpose and nursing responsibility. I can't find any of it in the net. PLEASE HELP ME!!

end of shift report is giving pertinent information about what has happened during your time with the patient to the oncoming shift of nurses. what i always did was quickly review what the doctor had ordered to be done (this is on the kardex) and how the patient did during my shift. for example, if lab or x-rays had been ordered report if they were done or not and the results of them if you know. if the patient is to receive pt twice a day mention that pt was at the bedside doing a treatment and the patient did well or was not cooperative. if dressing changes were ordered to be done qshift let the oncoming shift know when you did your dressing change and what the wound looked like. if the patient has an iv mention where the iv is and that it is patent or if you had to restart it. get the idea? you are just telling them what happened during your tour of duty. if there is an impending problem you pass that on as well. for example, you might have put a call into the doctor to ask for a stronger pain medication and haven't heard back from him yet. the oncoming nurse needs to know that.
