This is a new thing for the school where the student and preceptor complete an electronic log of goals and examples to meet the objective. I am not sure if I am filling it out correctly and an unsure of other goals I could use. Any suggestions?

Have you discussed it with your preceptor? Seems like you should be identifying what your goals are and the preceptor should be making a plan to achieve those goals....
