Well I sent my application in July but thought the the Hep A for both doses was starting in the spring 2017 not knowing I needed them both in order to apply. I got an email stating that I could still apply but either needed the Hep A second dose, which wasn't due until Oct 16, which was too late or try for a titer. I did a titer on September 30 and faxed to sentry before Oct 7, which was considered the late filing period

Yes, i sent it with when I sent my vaccinations

Oh I see! I think I didn't include mine when I sent my documents! Do you know if I would I have to repay the 15$ fee or just sent the proof with my old receipt to sentry?

No you dont have to repay the $15. Juse submit your insurance info and let them know you need to include it with your other documents
